This is the tool we use; we identify all of the examples we observe during the assessments, just started using the teacher's names, we used to just say 'Teacher' but when we put the actual teacher's name it gives them the recognition of what they are doing. The second part if the areas to work on, we generally just pick a few areas to focus on. If it is self-talk for example, we give them ideas how to do that as well as attach a simple handout with suggestions/ideas.
