After you have done a CLASS observation and you want to give some constructive feedback to the teachers, do you use a written form?  We have created a few but I was interested in others.

Last year our observer (me) didn't get to give the feedback face-to-face with all the teaching teams. Because of that, I made sure I had a detailed report for the team to review with their supervisor. I've attached a sample of what worked for us - kind of long, but it effectively let the teams know exactly what they did well, and what they could work on getting even better at.
