Hello I have a question, for the portfolio they ask for an emergency form. In my center, when I asked them for it, they gave an emergency plan, but one of the teachers of the courses said it was an emergency form contact. So I don't know which would be the right one, I hope you can help me. Thank you.

Thanks for reaching out! I asked our CDA team and they let me know that the emergency form mentioned in the CDA Competency Standards book is one of "Three samples of Record Keeping Forms" - so the correct form is the emergency contact form. I hope this helps!
