    Hello everyone. Does anyone have a feedback form they could share with me? I'm looking to do some observations at some sites and would like a form to give back to the staff regarding things that went well and goals to work on. Any support you have would be great.

    I have one that I created. I share the scores for each cycle and area. At the bottom I share something that was done well and then something that could use improvement or growth.

    Here is a form we created.  This allows us to share specific observations on how the score was determined and a place to collaborate ideas to identify teachers strengths in each dimension to have interactions them more often, for longer periods or even better! :)

    Good morning! Attached are two forms that we currently use in order to provide and gather feedback regarding CLASS observations. The CLASS Self-Reflection form is a form that the teaching teams fill out after their CLASS observation but before their CLASS reflection time with the observer. The CLASS feedback form is a form that the observer uses to give examples of the teaching team's strengths and to give suggestions for ways to improve the child-adult interactions in the classroom. Examples and suggestions are given for each dimension.  Hope this helps!
